Air about the lungs, Air out of the lungs, Air in the pleura, Primary Neumotórax, Secondary Neumotórax, Neumotórax for tension.
DEFINITION
The neumotórax is the air presence in the space pleural that is the serous layer that makes a detour to the lungs, which produces in turn a loss of the pulmonary volume.
TYPES OF NEUMOTÓRAX
The most frequent is the spontaneous neumotórax, which takes place without a determinant clear cause, one believes that small air bustle is caused by the rupture of in the lung. This type of neumotórax happens with more frequency in thin and tall men between 20 and 40 years of age, and smokers that also familiar precedents of the illness.
Another type is the secondary spontaneous neumotórax to another pulmonary illness as there can be the chronic bronchitis, the asthma, the pulmonary fibrosis, the pulmonary emphysema the fibrosis quística, the tuberculosis, or inclusive the tosferina. The secondary neumotórax to traumatism owes to the break of the pleura and of the lung, with the consequent communication between both, like result of a penetrating wound for accident, or for aggression. The neumotórax for tension is caused by an excessive increase of the pressure about the lung forcing it to collapse.
SYMPTOMS
The neumotórax typically produces a sudden sharp thoracic pain that deteriorates especially with the deep respiration or for the cough, collaborates to difficulty to breathe, the thoracic oppression appears. A second time to the absence of pulmonary air the cardiac frequency increases and there can appear a bluish color of the skin caused by the absence of oxygen. The picture generates in the patient an anxiety picture since it seems that he cannot breathe, and there can be sensation of sickness or decrease of the blood pressure.
DIAGNOSIS
In the pulmonary sounding his doctor will be able to observe a decrease of the respiratory noises in the affected side. After this find it will request him a few complementary explorations:
- Thorax radiography to determine the presence and air quantity in the pleura.
- Arterial Gasometría
TREATMENT
Since air exists in the pleura the treatment will consist of eliminating it of this space allowing this way the expansion of the lung.
If the air pleural is scarce it is possible to resolve only with rest.
If the air is abundant and compresses the lung there is realized the laying of a thoracic pipe between the ribs inside the space pleural, that allows the evacuation of the air from there towards the exterior.
This air drainage can last several days, for it the hospitalization is usually n
