CONCEPT
The dermatitis of the nappy is an eruption that appears in an anatomical area especially exposed to irritating substances:
- In the area of the nappy of the child there join factors of occlusion, friction, moisture, and the irritating direct action of the urine (ph from 5 to 8) on the cutaneous surface.
- The dregs also play an irritating role due to diverse enzymes that they contain.
- Also it informs the flora microbiana of the area, especially the Candid yeast albicans.
The result of all these combined factors is an aggression continued in the skin that ends up by producing an alteration of the cutaneous barrier, with red areas in the areas of major contact with the nappy, which soon erode and sobreinfectan.
DIAGNOSIS
Sometimes it is complicated for the pediatrician, since the dermatitis of the nappy can collaborate to other eruptions of the area:
- Dermatitis seborreica
- Dermatitis atópica
- Allergic contact Dermatitis
- Psoriasis...
TREATMENT
- Avoidance of the irritating factors by means of a good hygiene.
- Use of disposable nappies, transpirantes and not occlusive.
- Frequent change of nappy.
- Cleanliness of the area with not abrasive antiseptic soaps.
- Protection of the skin with protective ointments that contain oxide of zinc (like barrier) and moisturizing-emollient substances.
- Treatment of the dermatitis established with corticoids in cream of low potency (hidrocortisona to 1 %).
- Local treatment of the sobreinfección for yeasts or bacteria.
- When an established dermatitis exists, also it is of big help to remove the nappy and to make the area exhibited outdoors during all the possible time.
